Have this ready: your last day worked, your expected return date, and your treating provider's name, phone and fax. MetLife sends a statement to your provider to complete, and your claim cannot be approved until that comes back, so it is worth calling your doctor's office to nudge it.
What short-term disability pays: 60% of your weekly income, up to $1,000 a week, for up to 11 weeks. Payments begin on the 15th day of your illness or injury. Because the company grosses up the premium, the benefit reaches you tax free.
Still out past 90 days? Long-term disability takes over on day 91 at 60% of monthly income. Depending on your role, the company either pays for that coverage or you can buy it through payroll. If you bought it, a pre-existing condition treated in the three months before your coverage started is excluded for the first 12 months.
